摘要
目的:探讨新生儿20项神经行为测定(neonatalbehavioralneurologicalas-sessment,NBNA)的影响因素。方法:应用鲍秀兰等建立的新生儿20项行为神经评价方法于足月儿出生后、早产儿纠正胎龄40周后第3,7,14,28d分别进行80例次NBNA测定,每次NBNA测定前进行血糖,血清K+,Na+,Cl-,Ca2+,P2-,血红蛋白、尿素氮、肌酐、总胆红素、未结合胆红素、乳酸脱氢酶(lacticdehydrogenase,LDH)、肌酸激酶、肌红蛋白等检查,同时测量身长、体质量。结果:胎龄、体质量、血糖、血Na+与NBNA评分呈正相关(r=0.76,0.52,0.38,0.46)。血清总胆红素、未结合胆红素、LDH、肌酸激酶、尿素氮和肌酐与NBNA评分呈负相关(r=-0.72,-0.73,-0.34,-0.39,-0.28,-0.25),Apgar评分与NBNA评分密切相关(r=0.78)。结论:胎龄、体质量、Apgar评分、高胆红素血症、血糖、血钠、乳酸脱氢酶和肌酸激酶水平与NBNA评分及新生儿神经伤残密切相关。
AIM:To explore the influencing factor of twenty item neonatal behavioral neurological assessment(NBNA). METHODS:Totally 80 infants were assessed with the twenty item NBNA established by Bao XL,et al.at 3rd,7th,14th,28th day post birth of mature infants and after correcting age 40 weeks of preterm infants,respectively.Blood glucose,serum K+,Na+, Cl-, Ca2+,P2-,haemoglobin,blood urea nitrogen (BUN),serum creatinine(Scr),serum total bilirubin,unconjugated bilirubin, lactic dehydrogenase(LDH),creatine kinase(CK),and myoglobin(MYO) were measured before NBNA,and body height and mass were also measured at the same time. RESULTS:NBNA score was positively correlated with gestational age,body mass,blood glucose and serum Na+(r=0.76,0.52,0.38,0.46),While the NBNA score was negatively correlated with serum total bilirubin, unconjugated bilirubin,LDH,CK,BUN and Scr(r=-0.72,-0.73,-0.34,-0.39,-0.28,-0.25).Apgar score was closely related with NBNA score (r=0.78). CONCLUSION:Gestational age,body mass,Apgar score,hyperbilirubinemia,blood glucose,serum Na+are closely related with NBNA score and nervous disability in newborns.
