摘要
目的:研究运动治疗对充血性心力衰竭患者凝血功能的影响。方法:将57例充血性心力衰竭患者,分为运动组(28例)和对照组(29例),运动组进行运动康复治疗七八周;而对照组不进行运动康复治疗,观察实验前后的临床表现、血浆血管性假性血友病因子(vonWillebrandfactor,vWF)、P-选择素和D-二聚体水平变化。结果:两组实验前血浆vWF、P-选择素和D-二聚体水平无显著差异,运动组血浆vWF犤(138.96±33.84)%犦、P-选择素犤(19.02±3.97)g/L犦和D-二聚体犤(0.60±0.14)mg/L犦水平均较实验前犤(176.88±34.87)%,(24.46±4.32)g/L,(0.78±0.13)mg/L犦下降(t=4.12~4.98,P<0.05);对照组实验后血浆vWF、P-选择素和D-二聚体水平也较实验前下降(t=2.08~2.38,P<0.05)。实验后运动组血浆vWF、P-选择素和D-二聚体水平较对照组下降明显(t=2.12~2.41,P<0.05)。结论:运动康复可改善充血性心力衰竭患者的凝血功能,可能在一定程度上减少充血性心力衰竭患者血栓事件的发生和发展。
AIM:To study the effect of exercise training on blood coagulation in patients with congestive heart failure. METHODS:Fifty seven patients with congestive heart failure were randomized into two groups:exercise training group(n=28) and control group(n=29).Patients in the training group received exercise training plan for 7-8 weeks,while patients in the control group did not do that.The clinical situation,changes of levels von Willebrand factor(vWF),plasma P selectin and D dimer were observed before and after experiment. RESULTS:The level of plasma vWF,P selectin and D dimer had no significant differences before experiment in both groups,however,in the exercise training group,the concentration of vWF level was significantly lowered from(176.88±34.87)%to(138.96±33.84)%,plasma P selectin was significantly lowered from(24.46±4.32)g/L to (19.02±3.97)g/L,D dimer was significantly lowered from[(0.78±0.13)mg/L] to[(0.60±0.14)mg/L](t=4.12-4.98,P< 0.05)respectively.In the control group,the levelsn of vWF,plasma P selectin and D dimer were lowered as compared with those before experiment(t=2.08-2.38,P< 0.05).The levels of vWF,plasma P selectin and D dimer in exercise training group were more significant lower than those of the control group(t=2.12-2.41,P< 0.05). CONCLUSION:Exercise training can improve the blood coagulation in patients with congestive heart failure,and reduce the occurrence and development of thrombus to a certain extent.
