氯胺酮复合麻醉对心血管的影响

摘要 对战伤伤员施行四肢再清创手术时,在吸氧条件下,采用氯胺酮—氟哌啶、芬太尼复合麻醉,使用呼吸气量计(西德Drager)连接于国产103型麻醉机回路呼气侧观察通气量。用江西HL—3B型多用恒流阻抗仪观察心功能变化,以明确氯胺酮对呼吸的抑制与对心血管功能的双相作用是否存在着因果关系,即心血管副反应是否因缺氧所致。结果表明:在自主呼吸,吸100%氧,PaO_2 407.37±14.036mmHg,虽可避免低氧血症,但未能改善氯胺酮对心血管所产生兴奋及抑制的双相作用。 The influence on both respiratory and cardiovascular systems during Sodium pentothal, droperidol and fentanyl complex anesthesia was studied under the condition of oxygen inhalation for secondary debridement of limbs shell wounds. A Drager ventilomater made in West Germany was adopted and connected with the expiratory end of the anesthesia machine(type 103, made in China) to observe the ventilation, At the same time,a HL-3B cardiac impedance machine (made in Jangxi province)was used to observe the changes of cardiac function. The results demonstrated that while pure oxygen was taken in (8 L/min),it was able to remain PaO_2 at 407.37±14.036mmHg and avoid hypoxemia. But it could not reduce the double exciting and inhibiting effects of the drugs on eardiovaseular system.
