非溃疡性消化不良与幽门螺旋菌相关性慢性胃炎关系的研究

Non-Ulcer Dyspepsia and Helicobacter Pylori Associated Chronic Gastritis

在本院251例连续胃镜病例中,对非溃疡性消化不良(NUD)与幽门螺旋菌(HP)相关性慢性胃炎的关系进行了前瞻性研究。发现:(1)NUD的HP检出率与同期当地正常人群无显著性差异。(2)NUD之HP阳性与阴性组之临床症状计分无显著性差异;NUD典型组与非典型组间HP检出率无显著性差异。(3)HP感染及胃窦粘膜炎症程度与症状计分无显著相关性。(4)DeNol与安慰剂治疗比较,两组治疗后症状计分无显著差异。结果提示,NUD之临床症状与HP相关性慢性胃炎之间无明确关系。

A prospective study has been performed in 251 consecutive patients undergoing upper GI endoscopy in order to investigate the relationship between non-ulcer dyspepsia (NUD)and H. pylori(HP)associated chronic gastritis. Comparisons have been made as follows: (1)HP incidence in NUD group was not higher than that in age-matched group in general population. (2)Symptom scores between HP positive subgoup and HP negative subgroup were not significant, and HP incidence in patients with classic symptoms was not significantly different from tnat in patients with atypical aymptoms. (3)There was no significant correlation of symptom scores with the degree of HP density and histological severity of gastritis. (4)Clearance of HP was achieved in 20/21 cases treated with DeNol compared with 0/ 19 with placebo. However sympton scores remained no significant in these two groups before and after treatment. The results suggest that there is no correlation of symptoms in NUD to HP associated chronic gastritis.
