摘要
肾上腺疾病的磁共振成像(MRI)近年来屡有报道,本文阐述肾上腺正常与异常的MRI表现,以及MRI与CT对肾上腺肿瘤诊断价值的比较。检查方法:采用美国Diasonies 0.35T超导型磁共振仪,扫描序列为自旋回波(SE)T<sub>1</sub>加权(短TR,短TE)TR=250~500msec,TE=15~40msec,T<sub>2</sub>加权(长TR,长TE)TR=2000msec,TE=60~80msec,层厚0.5~1.0cm。T<sub>1</sub>加权图像显示肾上腺形态及其与周围结构的关系十分清晰,T<sub>2</sub>加权图像虽信噪比较差,但在判断病变时起到定性作用。只有T<sub>1</sub>与T<sub>2</sub>
The MRI characteristics of normal adrenal gland and adrenal masses were described. In 70 consecutive upper abdominal MR Cases without adrenal abnormalities were reviewed. In which 97.8% of right and 98.6% of left adrenal glands can be identified clearly on MRI. The right adrenal gland appeared as a linear shape structure in 77% and inverted'V' shape in 20%. The left adrenal gland was 'λ' shape in 44%; comma shape','20%;'\' shape 20%; inverted'V' shape 6%;'γ' shape 4%; small triangular shape 3%.The intensity of normal adrenal gland was lower than or similar to that of liver on both T_1 weighted (T-1W) and T_2 weighted (T_3W)images.Among the 17 proved cases of adrenal masses, two neuroblastomas (2/2); 3 ganglioneuromas (3/4); 2 pheochromocytomas (2/2); 1 adrenal cortieal eareinoma (1/2); 1 metastatic tumor (1/1);1 myelolipoma (1/1) and 2 cysts (2/2) were of very high intensities on T_3W images, whereas in 2 adenomas (2/2);1 adrenal cortical carcinoma (1/2); 1 adrenal hyperplasia (1/1) and 1 small ganglioneuroma (1/4 ), the intensity was visually isointense or slightly hyperintense as compared to that of liver on T_2W images.The authors considered that MRI is of value in characterizing an adrenal mass lesion.
