骨内高压减压前后血液流变学状态的实验研究(简报) 被引量:17

摘要 目前已知许多骨科疾病如骨性关节炎、缺血性骨坏死等均有骨内压异常增高。骨减压疗法是治疗骨内高压的一种有效方法。但骨内高压产生及治愈的机理目前尚不十分清楚。以往仅限于病理形态学和血流动力学方面的研究,应用血液流变学方法研究骨内压迄今尚未见有报道。由于骨内血液流变学和血流动力学状态是维持正常骨内微循环的基础。 It was known that there is an increased intraosseous pressure in various kinds of orthopedic diseases,and that decompression therapy can get excellent efficiency.The mechanism about how in- traosseous hypertention occurs and is cured isn’t yet clear.The previous studies are limited in patho- morphologic and hemodynimic aspects.The hemorheologic method used in this field hasn't been reported. The aim of the study is to find the hemorheological changes in both venous blood and in tramedullary blood before and after drilling decompression operation on the rabbit tibia with the in- traosseous hypertention. The results of the study are that:Immobilization of rabbit's knee in extent position for five weeks induces intraosseous hypertention of the proximal end of tibia,under this condition the viscosity of both whole blood and plasma is increased significantly,FSR become faster,RCD and electrophoresis rate of red blood cell is decreased,plasma fibrinogen is enhanced significantly. Drilling decompression decreases the intraosseous hypertention effectively,and the effect of the operation lasts for a long time, besides,it has an improved effect on the abnormal hemorheologieal states.In the experimental group,whole blood viscosity,ESR、RCD and red cell electrophoresis rate tends to return to normal,but in the control group,they do not return to normal.In both groups plasma viscosity and plasma fibrinogen doesn't alter after decompression. The results suggest that the hemorheological detection may be a new way for early diagnosis and may allow for commentary on the effect of the treatment on the intraossous hypertention,and that the hemorheological therapy may be a new method for the prevention and treatment of the intraosseous hypertention.The combination of decompression with hemorheological therapy may be more effective.
