带蒂胸锁乳突肌瓣移植在腮腺切除术中的应用

APPLICATION OF STERNOCLEIDOMASTOID MUSCLE FLAP TRANSPLANTATON DURING PAROTIDECTOMY

报告了14例腮腺全叶及肿瘤切除后,用带蒂胸锁乳突肌瓣移植,填塞于手术创口,对于防止副交感神经纤维的错位再生,预防味觉性出汗综合征,以及改善颌后区凹陷外观,均取得了较为显著的效果。经6个月至3年随访,其中只有1例尚存味觉性综合征,碘—淀粉试验阳性,惠者耳垂前下方凹陷畸形。余13例外观均无畸形,主观感觉正常。文中并介绍了手术方法,讨论了治疗机理。

Sternocleidomastoid muscle flap transplantation was applied to Solving the problem of parotidectomy-caused defect and gustatory sweating Syndrome in 14 cases.These patients were followed up for 0.5 to 3 years. Among them, only one patient Suffered from sweating syndrome and concave deformity, and all the remaining 13 cases were treated successfully by this surgical management. We introduced the operative method and discussed therapeutic principles in this article. The authors believe that this method can prevent malposed regeneration and anastomosis of amputated para-sympathetic nerve fibres, thus avoiding the gustatory sweating; and it can also ameliorate appearance of the maxillofacial concave deformity.
