高渗盐水对高原失血性休克心血管功能的改善作用

BENEFICIAL EFFECT OF HYPERTONIC SALINE ON CARDIOVASCULAR FUNCTION OF HEMORRHAGIC SHOCK AT SIMULATED HIGH ALTITUDE

摘要 高渗盐水用于治疗高原失血性休克,迄今未见报道。本文研究了少量高渗盐水对高原失血性休克家兔心血管功能的影响。在模拟4000m高原环境中,径股动脉放血,使平均动脉压降至6.0±0.27kPa,并维持60min。然后夹闭放血管,10min后经静脉分别输入相当于总失血量10%的高渗盐水(7.5%NaCl,高盐组,n=5)或等渗盐水(0.9%NaCl,等盐组,n=6),与治疗前或与等盐组相应值比较,高盐组家兔治疗后,平均动脉压左心室收缩压和左心室+dp/dt max均显著增高,提示高渗盐水能够明显改善高原失血性休克机体的心血管功能,有利于休克的复苏。 The effects of small amount of hypertonic saline on cardiovascular function of rabbits during hemorrhagic shock were studied in hypobaric chamber at a simulated altitude of 4000m. The animals wrer exsanguinated from femoral artery to reduce mean arterial pressure ure to 6.0kPa within 12 min, and kept at this level for the following 60 min. After that the bleeding catheter was clamped for 10 min and then the rabbits were given intravenous infusion of 7. 5%NaCl(2400mOsm/L) amounted to 10% volume of shed blood for test group and 0. 9% NaCl(300mOsm/L)for control one,respectively. Hemodynamic analysis showed that hypertonic saline significantly increased mean arterial pressure,left ventricular pressure and LV+dp/dt max. The same volume of isomotic saline had no obvious beneficial effect on cardiovascular function of shock rabbit. The results suggestec that at high altitude,the small amount of hypertonic saline was effective in improving the cardiovascular function of hemorrhangic shock as well as it did in normoxic condition.
