下颌骨骨折是颌面部常见的创伤,在治疗中维持良好的固定以恢复口腔功能是治疗的关键。下颌骨骨折的固定主要分为内、外固定二类,有关下颌骨骨折固定的方法和效果国内外已有大量的研究和报导。外固定中以颌间固定用得最多,它无需手术效果肯定,但是这种固定方法限制了病人的张口运动及进食,且对无牙(牙合)病人,精神病患者及伴有颅脑损伤的病人不适宜。内固定在下颌骨骨折的应用也很常见,近二十年来加压内固定在国外得到广泛的应用。
This study was undertaken to compare the influences and the results of internal fixation using NiTi shape memory alloy staples (SMAS) and stainless wire ligatures (SWL) after experimental mandibular fractures in 46 rabbits. We had taken the roentgenography, microangiogram, histological sections and scanning electric microscope to observe the healing process after 7, 14, 21, 30, 60 days. The results showed that more external callus were formed in SWL fixation site than that in SMAS fixation site. The healing process in SWL fixation was completed through cartilage to bony callus formation, while that in SMAS fixation was completed direct through bony callus formation. We measured the contents of calcium in the callus after 7, 14, 21, 30, 60 days. The calcium contents of callus in SMAS fixation was higher than that in SWL fixation at the same days. The calcium contents of callus in SMAS fixation after 30 days was 210.84±28.59mg/g, while that in SWL fixation was 169.53±14.51mg/g. The calcium contents of the callus in SMAS fixation after 60 days was 268.24±55.36mg/g, while that in SWL fixation was 167.30±28.79mg/g. The difference was significant (p<0.01). The biomechanics of the fracture healing was measured by bending force after 30, 60 days. The breaking strength after 30 days in SMAS fixation was 18.02±5.16 kg, while that in SWL fixation was 12.58±4.57kg. The breaking strength after 60 days in SMAS was 20.55±4.29kg, while that in SWL fixation was 13.90±3.98kg. The difference was signicant (p<0.01). From the results we had proved that SMAS fixation were more stable than SWL fixation. The use of NiTi shape memory alloy staples in mandibular fractures is easier in clinic and it is worthy of spreading in clinic use.
