30例手术前、后血浆加压素浓度的变化

摘要 用放免法观察30例病人手术前、后血浆加压素(AVP)浓度的变化。结果显示手术15min后AVP为18.42±17.71pg/ml,比麻醉前(6.84±6.13pg/ml)显著增高(P<0.01)。这组病人手术15min后的血浆β-内啡肽免疫活性物质(ir—β—EP)含量为623.64±468.22pg/ml。与麻醉前比较亦明显升高(P<0.01)。讨论认为麻醉与手术均为应激因子。在麻醉与手术的应激下,将通过血浆β-内啡肽的升高而刺激AVP释放。AVP增高可以导致血压增高、冠状动脉痉挛、心肌缺血与心律失常。这将对手术过程造成一定的危害,值得注意。 Radio-immuno-assay (RIA) method was used to observe changes in plasma vasopressin (AVP) concentration of 30 patients before and after surgical operations. The results showed that at 15 min after surgery the AVP value was 18.42 ± 17.71pg/ml, which was significantly higher than the pre-anaesthetic value of 6.84 ±6.13pg/ml (P<0.01). At the same time, the plasma ir-β-EP in this patient group was 623.64±468.22pg/ml, which was also significantly higher than its pre-anaesthetic value (P<0.01). It is considered that anaesthesia and surgery are both stress factors, and under these stimuli AVP will be released due to the increasing plasma β-EP. It should be noted that the increment of AVP can induce high blood pressure, coronary spasm, myocardial is chemia and cardiac arrhythmia, and therefore may bring considerable hazard to surgical procedures.
