尘肺病人肺功能分级研究

STUDY OF THE CLASSIFICATION OF PULMONARY DYSFUNCTION IN PNEUMOCONIOSIS PATIENTS

作者对425例尘肺病人和285例煤矿工人的肺通气功能(VC、FVC、FEV_1、FEV_1/FVC%)和肺弥散量(DL_(co))进行了测定和研究。以206例健康不接尘工人肺功能测定数据为样本,提出尘肺患者肺功能损害程度的分级标准,正常值范围:VC、FVC、DL_(co)≥80%,FEV_1≥75%,FEV_1/FVC≥70%;轻度损害:VC、FVC、DL_(co)65～79%、FEV_1 60～74%、FFV_1/FVC%55～69%;重度损害:VC、FVC、DL_(co)<50%,FEV_1<40%,FEV_1/FVC%<35%,用此标准检验425例尘肺病人。其结果较符合我国的实际情况。

Ventilatory function(VC,FVC,FEV1.FEV1/FVC%)and single breath diffusing capa- city (DLco)of 425 pneumoconiosis patients and 285 coal miners were examined.200 health workers without dust exposure were selected as a normal controls.The classification of the degree of respiratory impairment was suggested as follows:normal range:VC,FVC,DLco ≥80% of predicted,FEV1≥75% of predicted.FEV1/FVC≥70%:slight impairment:VC, FVC.DLco 65-79% of predicted,FEV1 80-74% of predicted,FEV1/FVC 55-69%;mode- rate impairment:VC,FVC,DLco 50-64% of predicted,FEV1 40-59% of predicted,FEV1/ FVC 35-54%;heavy impairment:VC,FVC,DLeo<50% of predieted,FEV1<40% of predi- cted.FEV1/FVC<35%.Meanwhile,the pulmonary function status of the 425 patients was assessed according to the criteria.It showed that the recommended criteria seem to be proper and suitable for pneumoconiosis in our country.
