蛇毒凝血试验诊断肝病凝血功能障碍的探讨

摘要 本文对127例各类肝病和55例正常人进行蕲蛇毒时间(AT)、蝗蛇毒磷脂时间(RVVCT)和蝰蛇毒血小板三因子活性(RVVPF_(3a))三项蛇毒凝血试验的研究。结果显示:AT、RVVCT和RVVPF_(3a)在各类肝病的测得值均显著高于正常人(P<0.001);肝病的异常检出率分别是93%(118/127)、74%(94/127)和81%(103/127),均显著高于凝血酶时间(TT)(63%),凝血酶原时间(PT)(34%)和白陶土部分凝血活酶时间(KPTT)(35%);三者的测得均值与不同期肝病有正相关关系,肝病越晚期,蛇毒凝血试验时间越延长。认为AT、RVVCT和RVVPF_(3a)能从三个不同侧面去反映肝病凝血功能障碍,其敏感性均优于TT、PT和KPTT,可以作为肝病凝血功能检查的新指标。 This paper reports the study of 3 marks of snake venom coagulant tests: Agkistrodon acutus Venom Time (AT), Russell's Viper Venom Cephalin Time (RVVC T) and Russell's Viper Venom Platelet Factor 3 availability (RVVPF-3a), in 127 patients with various hepatic diseasses and 55 normal subjects. The results showed : the values of AT, RVVCT and RVVPF-3a of various hepatic diseases were significantly higher than those in normal subjects (P<0.001). The detection rates of AT、RVVCT and RVVPF-3a in the hepatism were 93%, 74% and 81%, respectively, which were higher than those of Thrombin Time (TT) (63%), Prothrombin Time (PT) (34%) and Kaolin Partial Thromboplastin Time (KPTT) (35%). The mean values of 3 marks were positively correlated with the hepatic diseases in different stages. The more advanced the stage, the more prolonged the 3 marks. Therefore, it is suggested that AT, RVVCT and RVVPF -3a can reflect the abnormal blood coagulation function in hepatism from three different aspects, and are better than TT, PT and KPTT in their sensitivity. AT, RVVCT and RVVPF - 3a may be new marks in detecting the blood coagulant function in hepatism.
