应用显微外科技术治疗14例颅内动脉瘤的体会

摘要 应用显微神经外科技术直接治疗颅内动脉瘤是现代神经外科的重要手段之一。本组14例颅内动脉瘤手术连续成功的关键在于:术前须有显影充分的全脑血管造影,按Hunt和Hees分级选择手术病人,在显微镜下仔细分离动脉瘤颈部,选用合适的动脉夹夹闭,术后加强监护并使用Nimodipine治疗。 The application of microsurgical technique to the field of intracranial aneurysmal surgery is one of the effective treatment methods in modern neurosurgery. With the experiences in treating fourteen patients with direct clipping of the aneurysms, we have found that preoprative total angiography with adequate visualization of the aneurysms as well as their related vessels, selection of patients for surgery according to Hunt and Hess Grading, careful dissection of the aneurismal neck under operating microscope, selection of proper aneurysmal clips, intensive postoperative care and Nimodipine administration are prerequisites to successful surgery for intracranial aneurysm.
