保守手术和根治性放射治疗早期乳腺癌

RADIATION THERAPY FOR OPERABLE BREAST CANCER WITHOUT MASTECTOMY

早期乳腺癌保守手术和根治性放疗的疗效已达到与根治术相同的水平,在国外的应用日益扩大,国内还很少报道。我院自1958年3月至1983年底有22例早期乳腺癌病人,作了乳腺区段切除术及根治性放疗。从1985年8月起开始有计划地对部分Ⅰ、Ⅱ期乳腺癌病人进行保守手术和根治性放疗的临床研究,至1988年底共收治39例。

Sixty-two operable breast cancers in 61 patients (one patient with bilateral primary breast cancers) were treated by conservative surgery and definitive radiotherapy from March 1958 to December 1988. It consisted of tumorectomy or segmental mastectomy with or without axillary dissection followed by radical radiotherapy to the breast, axillary, supraclavicular and internal mammary node regions. The local-regional relapse rate was 4.8% and the distant metastasis rate was 12.9%. The 5-, 10- and 15-year actuarial survival rates were 80.2%, 76.7% and 66.79%, respectively. The results presented show that conservative surgery and definitive radiotherapy is as effective as radical mastectomy for early breast cancer but superior in functional and cosmetic results
