乳腺癌根治术后复发病例分析

PATTERNS OF POST-OPERATIVE RECURRENCE OF BREAST CANCER

乳腺癌为女性常见恶性肿瘤,手术治疗为目前治疗的主要方法,但关于乳腺癌根治术后放射治疗的适应和价值,仍有不同看法。本文通过对我科治疗的乳腺癌中根治术后复发的203例进行分析,探讨乳腺癌根治术后放射治疗的适应证和价值。

The patterns of 203 patients suffering from postoperative recurrent breast cancers is presented. 139 patients had received radical mastectomy only (operative group) whereas 64 received regional lymph node irradiation after surgery (post-operative irradiation group). The results showed: 1. Recurrence rate within 1 year in the operative group was higher than that of the post-operative irradiation group (P=0.01), 2. Ipsilateral supraclavicular metastasis was lower in the post-operative irradiation group (P<0.005), 3. Multiple region recurrences were more common in the operative group than in the post-operative irradiation group (P≤0.05) ,and 4. No difference in the recurrence rates in the ipsilateral axilla and chest wall in both groups (P>0.05). We suggest that elective postoperative irradiation of the ipsilateral supraclavicular region should be performed in Stages Ⅱ and Ⅲ breast cancers after radical mastectomy and elective irradiation of the ipsilateral axilla and chest wall is indicated only in selected patients.
