食管未分化癌的治疗

TREATMENT OF ANAPLASTIC CARCINOMA OF THE ESOPHAGUS

食管未分化癌较少见,国内外都只有零星散在的报告。1958～1989年我院收治了43例。本文总结它的临床特点,生物特性,治疗方法和效果。

From 1958 to 1989, 43 patients with anaplastic carcinoma of the esophagus as proved by pathology or cytology were treated. 14 patients were treated by surgery alone, 25 by radiotherapy alone, 1 by chemotherapy alone and 3 by chemotherapy combined with radiotherapy. The median survival and 1-year survival rate were as follows. 8 months and 25% (11/43)in the whole series, 7.6 months and 21% (3/14) for the surgery alone group, 6.8 months and 24% (6/25) for the radiotherapy alone group, 16 months and 2/3 for the chemo-radiotherapy group. The other patients who had survived for 8 months after chemotherapy died of local uncontrol. This tumor differs from the ordinary eso-phageal squamous cell carcinoma in that it fails chiefly from distant metastasis. Local recurrence and uncontrol stood second. The authors believe that satisfactory results may be obtained by the combination of radiation and systemic chemotherapy.
