体外循环心脏手术后乳糜胸

Chylothorax complicated cardiac surgery with extracorpreal circulation

本文报告了5年中经胸骨正中切口行体外循环心脏直视手术后并发乳糜胸8例。患者均为较重的法乐四联症,其中7例年龄<10岁,6例发生术后心脏低排出量。经放置闭式引流管、胸腔内注入四环素、控制饮食和治疗心衰等措施,所有患者全部治愈。作者认为对这种类型的乳糜胸应以保守治疗为主。

A total of 8 cases with chylothorax as the complication of extracorporeal circulation through median sternotomy is reported.All patients aged <10 years except one case,were diagnosis pre-and postope- ratively as-severe tetralogy of Fallot and 6 of them occurred low cardiac output syndrome immediately after operation.All patients with chylotho- rax were cured by intercostal pleural drainage,tetracycline pleurodesis, dietary modification and effective treatment of heart failure.Authors pre- feted the conservative management for this kind of chylothorax.
