摘要
1959年6月～1990年4月共施行闭式二尖瓣扩张术90例(其中1986年以后85例),无手术死亡。体会:严格选择手术适应症、术前维持心功能在最佳状态,是降低病死率和减少各种并发症的重要措施。经心尖单孔直法扩张,是左胸入路时左心耳因血栓机化而无法进入的可行性方法之一。瓣口扩张前后左房压测定有助于了解瓣口狭窄解除程度以及有无关闭不全。
This paper reports the experience of closed mitral valvotomy in 90 cases. There was no operative deach. Strict adoption of operative indication and maintaince of the cardiac function in better condition are very important before operation. It is better to dilate the major axis of effective orifice to 3.0～3.2 cm. In case that in the left auricl thrombus organization make the operation not to carry out through the left thoracot omy, direct dilatation of the stenotic mitral valve through the cardiac apex approch is one of the feasible operations. Measurement of the left atrical pressure can help operator to evaluate the condition of functive improvement and to judge if the mitral regurgilation occurs.
