幽门螺旋菌与胃粘膜组织病理学的初步研究

The relationships between the gastric diseases and the pyloric spiroid bacteria

对693例各种胃疾病的患者进行了胃枯膜病理学、微生物学及其与临床关系的研究,探讨幽门螺旋菌的致病作用。结果:98份样本中,培养出幽门螺旋菌59例,其中慢性胃炎阳性率62.7％ (浅表性胃炎阳性率最高,达91.1％),消化性溃疡阳性率57.4％;在慢性胃炎患者胃窦粘膜病理切片中可检出,仍以慢性浅表性胃炎检出率最高达91.1％。15例幽门螺旋菌阳性消化性的溃疡患者,经TDB治疗4周后有12例细菌消失,并伴有组织形态学改变。幽门螺旋菌感染与胃粘膜浸润程度有密切关系。本项研究表明,幽门螺旋菌在慢性胃炎和消化性溃疡中分离阳性率较高,单纯浅表性胃炎最高。临床对消化性溃疡TDB治疗有效。

The pathology and the bacteriology of the gastric mucosa, as well as their relationship with the clinical manifestations were studied in 693 patients with various gastric diseases. The results had positive cultures of the pyloric spiroid bacteria in 59 of 98 samples. The cultures of the spiroid bacteria were positive in 62.7％ of the patients with chronic gastritis (including 91.3％ of the patients with superficial gastritis), and 57.4％ of the patients with peptic ulcer. The same organisms may be found also in the pathologic slices, and the highest positive percentage was also in the patients with chronic superficial gastritis (91％). The spiroid bacteria disappeared and the histological, morphological conditions were improved in 12 of the 15 patients with peptic ulcer and positive culture of the pyioric spiroid baeteria, after the patients were treated by TDB for 4 weeks. This study shows that the pathological alterations of the gastric mucosa were in close relationship with the infection of the pyloric spiroid bacteria. The pylorie spiroid bacteria were isolated at a high percentage in the patients with chronic gastritis or peptic ulcer, and the highest percentage was in the patients with the simple superficial gastritis, The treatment with TDB was effective for the patients with peptic ulcer.
