改良菱形皮瓣修复唇周皮肤缺损50例

50 Cases with Skin Defects Near Lip Repairment with Modified Rhomboid Flap

目的:观察改良菱形皮瓣修复唇周皮肤缺损的效果。方法:近5年来我科应用改良菱形皮瓣修复唇周病变切除后皮肤缺损50例。修复缺损创面大小直径3~30mm。结果:所有皮瓣均一期愈合。应用改良菱形皮瓣修复5mm以下唇周皮肤缺损时,皮瓣的切取、缝合稍有困难;靠近唇缘的皮肤缺损在5~20mm时,改良菱形皮瓣的旋转、缝合较容易,且术后唇、鼻无继发畸形;超过20mm的皮肤缺损,尤其是上唇,用改良菱形皮瓣修复后可出现轻微的唇、鼻畸形。结论:改良菱形皮瓣在唇周中等大小皮肤缺损的修复具有明显优势。

Objective:To observe the effect of reconstruction of skin defect near lip with modified rhomboid flap. Methods:Reviewing the 50 cases under the application of modified rhomboid flap for repairing skin defects near lip in recent 5 years. The defect sizes were from 3 mm to 30 mm in diameter. Results:All flaps survived with primary healing. There are some difficulties in cutting and suturing modified rhomboid flap, when the skin defect near lip was being repaired, as the diameter of skin defect is less than 5 mm. As skin defect in size from 5 mm diameter to 20 mm, it is easy to remove and to suture. The lip and the nose do not become abnormally after surgery. As the size of defect exceed 20 mm diameter, particularly lay on upper lip, it could make the lip and nose to be slight abnormal. Conclusion:Modified rhomboid flap has obvious advantages in the medium-sized skin defect near lip.
