放化疗对鼻咽癌患者淋巴细胞亚群影响的观察

Observation of influence of radiotherapy and chemotherapy on T Lymphocytes subsets in patients with nasopharyngeal carcinoma

目的:观察鼻咽癌患者放化疗前、后的细胞免疫功能变化,探讨放化疗对鼻咽癌患者免疫功能的影响。方法:应用FACS Calibur流式细胞仪对125名健康体检者和148例鼻咽癌患者放化疗前、后外周血T、B淋巴细胞亚群进行检测,并分别对健康体检者与鼻咽癌患者、鼻咽癌患者放化疗前与放化疗后的检测值进行比较。结果:鼻咽癌患者放疗前与健康体检者对比,CD3+、CD16+56+和CD19+均值差异无统计学意义,t值分别为1.657 1、0.791 0和1.516 6,P值分别为0.098 7、0.429 6和0.130 5。CD4+、CD4+/CD8+均值下降差异有统计学意义,t值分别为2.398 5和2.385 2,P值分别为0.017 1和0.017 8。CD8+均值升高差异有统计学意义,t=-2.333 7,P=0.020 3。鼻咽癌患者放疗后与放疗前对比,CD3+、CD4+、CD4+/CD8+均值明显下降差异有统计学意义,t值分别为-7.361 2、-11.239 9和-8.271 8,P值均<0.01;CD8+与CD16+56+均值明显上升,差异有统计学意义,t值分别为3.817 8和4.833 8,P值分别为0.000 2和0.001。CD19+均值差异无统计学意义,t=-0.968 0,P=0.333 8。结论:鼻咽癌患者免疫功能下降,外周血T、B淋巴细胞亚群的检测对判断患者的免疫功能有参考作用。

OBJECTIVE:To explore the relationships between changes of immune functions in patients with nasopharyngeal carcinoma(NPC) pre-chemoradiotherapy and post-chemoradiotherapy.METHODS:The flow cytometry(FACS Calibur) was used to detect the blood T,B Lymphocytes subsets of 125 healthy check-ups and 148 NPC patients before and after chemoradiotherapy.To analyze the immune indexes of changes before and after the radiotherapy,then compare their test values.RESULTS:There were no significant differences in CD3+,CD16+56+,CD19+(t were 1.657 1,0.791 0,1.516 6,P were 0.098 7,0.429 6,0.130 5)between the healthy check-ups and the NPC patients before chemoradiotherapy.CD4+、CD4+/CD8+ had a downward trend(t were 2.398 5,2.385 2,P were 0.017 1,0.017 8) while CD8+ had an upward trend(t=-2.333 7,P=0.020 3),which was statistically significant.CD3+,CD4+,CD4+/CD8+ were significantly different before and after the chemoradiotherapy,CD3+、CD4+、CD4+/CD8+gradually down,which had significant differences(t were-7.361 2,-11.239 9,-8.271 8,P<0.01);CD8+、CD16+56+gradually up,which had significant differences(t were 3.817 8,4.833 8,P were 0.000 2,0.001).There was no significant difference in CD19+(t=-0.968 0,P=0.333 8).CONCLUSIONS:The immune functions get lower in the NPC patients after chemoradiotherapy and can be estimated by the test of the blood T,B Lymphocytes subsets.
