冠心病患者内皮细胞微粒水平与Th1/Th2细胞因子水平的关系研究 被引量:7

摘要 目的探讨冠心病患者外周血内皮细胞微粒(EMPs)水平与辅助性T细胞(Th)1/Th2细胞因子水平的关系。方法选取2011年入住我院的稳定性心绞痛患者27例(SAP组)、不稳定性心绞痛患者28例(UAP组)及急性心肌梗死患者30例(AMI组),同时选择同期冠状动脉造影检查正常的住院患者20例作为对照组。检测各组血浆EMPs水平及血清中的γ干扰素(IFN-γ)、白介素(IL)-2、IL-4和IL-10水平。分析EMPs水平与Th1/Th2细胞因子水平的相关性。结果 4组受检者EMPs水平间差异有统计学意义(P=0.000),其中UA组和AMI组血浆EMPs水平明显高于对照组和SA组,SA组明显高于对照组,AMI组明显高于UA组,差异均有统计学意义(P<0.05)。4组受检者的血清IFN-γ和IL-2水平间差异均有统计学意义(P=0.000),其中UA组和AMI组血清IFN-γ和IL-2水平明显高于对照组和SA组,SA组血清IFN-γ和IL-2水平明显高于对照组,差异均有统计学意义(P<0.05)。4组受检者的血清IL-4和IL-10水平间差异均无统计学意义(P>0.05)。EMPs水平与IFN-γ和IL-2水平呈正相关(r=0.795,P=0.000;r=0.804,P=0.000),与IL-4和IL-10水平无直线相关性(r=-0.088,P=0.374;r=-0.156,P=0.112)。结论 EMPs可能参与冠状动脉粥样硬化的免疫和炎症的调控过程,且EMPs和Th1细胞因子水平变化可能与冠状动脉不稳定性斑块相关。 Objective To investigate the relationship between endothelial microparticles(EMPs) and the levels of Th1,Th2 cytokines in patients with coronary heart disease(CHD).Methods The levels of EMPs,IFN-γ,IL-2,IL-4 and IL-10 were measured in 27 patients with stable angina(SA group),28 with unstable angina(UA group),30 with acute myocardial infarction(AMI group),and 20 inpatients with normal conventional coronary angiography(control group).The correlation of EMPs with Th1,Th2 cytokines was analyzed.Results There was significant difference in levels of EMPs between 4 groups(P=0.000),thereinto groups UA,AMI higher than groups control,SA group higher than control group,AMI group higher than UA group,the difference was significant(P<0.05).There was significant difference in IFN-γ,IL-2 between 4 groups(P=0.000),thereinto groups UA,AMI higher than groups control,SA;SA group was higher than control group,the difference was significant(P<0.05).No significant difference was noted in IL-4,IL-10 between 4 groups(P>0.05).EMPs were positively correlated with IFN-γ,IL-2(r=0.795,P=0.000;r=0.804,P=0.000),not correlated with IL-4 and IL-10(r=-0.088,P=0.374;r=-0.156,P=0.112).Conclusion EMPs may be involved in the regulation process of immunity and inflammation of coronary atherosclerosis.The changes of the levels of EMPs and Th1 cytokines may be correlated with unstable coronary atherosclerotic plaque.
