摘要
目的探讨高龄患者经皮内镜下胃/空肠造口术(PEG/J)肠内营养支持的应用及系统管理方法。方法对29例高龄老年PEG/J术肠内营养支持患者实施系统化管理,观察分析患者并发症、营养状态的变化,评价肠内营养护理管理质量。结果本组老年患者PEG/J术无置管相关的严重并发症,与术前相比,PEG/J术后吸入性肺炎次数减少(P<0.05),营养状况不同程度改善(P<0.05);实施系统化管理前后肠内营养专项护理质量及患者满意度差异有统计学意义(P<0.01)。结论 PEG/J是高龄老年患者安全可靠的肠内营养支持途径,实施系统化管理可减少肠内营养相关并发症,提高高龄患者长期肠内营养的安全性,并提高肠内营养护理质量和患者满意度。
Objective To explore the application and systematic management of enteral nutrition support for senile patients with percutaneous endoscopic gastrostomy/jejumostomy(PEG/J) in geriatric ward.Methods 29 senile PEG/J patients with enteral nutritional support were conducted with systematic management,and complications,change of nutritional status and quality of nursing for enteral nutritional support were observed and analyzed.Results There were no severe catheterization-associated complications detected in these senile patients.Frequency of inhalation pneumonia after operation was less than that before operation(P<0.05),and nutritional status of patients was improved as well(P<0.05).There were significant differences of quality of nursing for enteral nutrition support and satisfactory degree of paitents before and after the implementation of systematic management(P<0.01).Conclusion PEG/J is a safe and reliable approach for enteral nutrition support in senile patients.Implementation of systematic management can reduce the incidence of related complications,improve security of long-term enteral nutrition support in senile patients and improve the quality of nursing for enteral nutrition support as well as the satisfactory degree of patients.
