颅内动脉瘤血管内介入栓塞治疗术中预防出血并发症的经验探讨

The empirical approach on prevention of hemorrhage complication during intracranial aneurysm endovascular interventional embolization

目的探讨颅内动脉瘤血管内介入栓塞治疗术中出血并发症的预防。方法回顾性分析我院2009-02—2011-02收治的20例颅内动脉瘤血管内介入栓塞治疗中出现再破裂出血患者的防治方法。结果在手术操作中,再破裂出血患者行快速栓塞术,10例即时行开颅清除血肿术,6例行侧脑室外引流术,4例行腰大池外引流术。术后9例恢复良好,6例出现轻度的瘫痪,2例重残,3例死亡。结论在颅内动脉瘤血管内介入栓塞治疗中,规范操作能够有效降低出血并发症,提高治疗效果,对出血并发症患者及时、有效地处理非常重要。

Objective To investigate the clinical experience on prevention of hemorrhage complication during intracranial aneurysm endovascular interventional embolization.Methods Retrospective investigation and analysis were carried out in 20 patients with rebleeding from cerebral artery rupture after intracranial aneurysm endovascular interventional embolization from February 2009 to February 2011 in our hospital.Results The rapid embolization were performed in patients with rebleeding during the operation,10 patients underwent craniotomy evacuation of hematoma,among them,6 cases were lateral cerebral ventricular drainage,4 cases were lumbar cistern drainage.9 patients recovered well after operation,6 cases showed mild paralysis,2 patients with severe disability,3 patients died.Conclusion Standard operating procedures can effectively reduce bleeding complications during intracranial aneurysm endovascular interventional embolization,so as to improve the clinical treatment effect,and the timely,effective treatment is very important to the patients with bleeding complications.
