急性髓系白血病患者SALL4基因的甲基化改变及其临床意义

摘要 目的:探讨急性髓系白血病(acute myeloid leukemia,AML)患者婆罗双树样基因4(Sal-like 4,SALL4)启动子的甲基化状况和临床意义。方法:分别应用甲基化特异性PCR(methylation-specific PCR,MSP)技术和实时定量PCR(RQ-PCR)技术检测45例初发AML患者和20例缺铁性贫血患者骨髓标本的SALL4甲基化状态和SALL4表达水平;同时,对AML患者的SALL4甲基化改变与临床特征相关性进行分析。结果:9例(20.0%)AML患者存在SALL4基因启动子低甲基化改变,而20例对照均无SALL4基因低甲基化,两组比较差异有统计学意义(P<0.05)。SALL4启动子低甲基化改变与AML患者年龄、性别、血红蛋白含量、血小板计数均无相关性(P>0.05),但低甲基化患者的白细胞计数明显高于甲基化患者(P=0.027)。SALL4表达水平和SALL4低甲基化明显相关(P<0.05)。按WHO分型,本研究中共有7类亚型,亚型间SALL4低甲基化频率差异无统计学意义(P>0.05)。中危和高危组核型患者中SALL4低甲基化频率明显高于低危组患者(P<0.05)。结论:SALL4低甲基化是AML患者中一个常见分子事件,可能与AML患者中高危核型相关。
