胃癌手术的现况与争议

摘要 我国是胃癌的高发区,是我国肿瘤相关性死亡最常见的病因之一.2008年国际抗癌组织公布的资料显示,我国男性或女性胃癌患者的标准化死亡率均高居世界的第二位,分别为30.1/10,000和14.6/10,000[1].手术仍然是胃癌的唯一根治手段.1881年,Billroth成功开展第一例胃癌局部切除+胃空肠吻合术,开启了胃癌手术切除的先河,并开始了长达80年的胃癌治疗模式,即肿瘤病灶切除+消化道重建.至20世纪60年代,日本学者对胃癌的淋巴转移研究更加深入,逐渐肯定胃癌的转移以淋巴转移为主,并提出规范化淋巴结清扫的概念[2],促使进展期胃癌患者的生存获得大大提高.
