重症监护病房呼吸机相关性肺炎的危险因素分析

摘要 目的重症监护病房(ICU)呼吸机相关性肺炎(VAP)发生的相关危险因素,为探讨VAP的预防提供参考。方法选取我院重症监护病房2012年3月到2013年4月所收治的80例行人工机械通气的ICU住院患者,对VAP发生的相关危险因素进行Logistic回归分析,筛选VAP发生的相关危险因素。结果本组80例呼吸机相关性肺炎患者中,死亡16例(20.0%),33例(41.3%)发生VAP。VAP发生的危险因素主要是误吸、机械通气时间≥3天、抗菌素联合应用≥2种。这充分说明,VAP的发生与误吸、机械通气时间、抗菌素使用有关。结论 VAP发生的危险因素为昏迷、误吸、机械通气时间≥3天、抗菌素联合应用≥2种,如果合理控制、重点评估这些危险因素,是能够大幅度降低VAP的发生。 Objective To investigate the risk factors of incidence of of incidence of ventilater associated pneumonia (VAP) in ICU and provide reference for prevention of VAP. Methods A hospital intensive care unit in March 2012 to April 2013 are treated in 80 mechanical y ventilated ICU routine artificial hospitalized patients on risk factors of VAP Logistic regression analysis, screening risk factors of VAP. Results 80 cases of ventilator-associated pneumonia patients, 16 patients died (20.0%), 33 cases (41.3%) occurred VAP. The main risk factors of VAP is aspiration, duration of mechanical ventilation ≥3 days, antibiotics combined with ≥2specTes. This ful y shows that, VAP occurrence and aspiration, duration of mechanical ventilation, use of antibiotics. Conclusion VAP risk factors for the coma, aspiration, duration of mechanical ventilation ≥3 days, antibiotics combined with ≥2 species, if reasonable control, focusing on assessment of these risk factors, is the ability to significantly reduce the incidence of VAP.
