化痰活血方穴位贴敷合西药治疗强直性脊柱炎临床研究

Clinical research on acupoint sticking with Huatanhuoxue prescription combined with western medication for ankylosing spondylitis

目的:观察化痰活血方穴位贴敷联合西药治疗强直性脊柱炎的临床疗效及其安全性。方法:将80例强直性脊柱炎患者随机分为观察组(43例)和对照组(37例)。对照组使用西药治疗,口服萘普生缓释片与柳氮磺吡啶肠溶片,共治疗6个月。观察组在对照组的基础上采用化痰活血方穴位贴敷,使用自拟配方中药制成药饼贴敷于膀胱俞、肾俞、膈俞、环跳,阳陵泉、脾俞、关元俞、大杼,每次贴敷4小时,一周治疗1次,共治疗6个月。观察治疗前后患者的视觉模拟评分(VAS)、Bath强直性脊柱炎疾病活动性指数(BASDAI)、Bath强直性脊柱炎功能指数(BASFI)、血沉(ESR)、C反应蛋白(RP)以及在治疗期间出现的不良反应情况,并评定两组临床疗效。结果:观察组总有效率81.4%(35/43),优于对照组的59.5%(22/37,P<0.05)。治疗后两组患者VAS、BASDAI、BASFI评分较治疗前显著降低,且观察组比对照组更为明显(均P<0.05)。两组患者治疗后ESR、CRP较治疗前显著降低(均P<0.05),但两组间比较差异无统计学意义(均P>0.05)。结论:化痰活血方穴位贴敷联合西药治疗强直性脊柱炎安全、有效,尤其在缓解疼痛、减轻晨僵、改善脊柱功能方面具有明显优势。

Objective To observe clinical efficacy and safety of acupoint sticking with Huatanhuoxue prescription combined with western medication for ankylosing spondylitis(AS).Methods Eighty cases of AS were randomly divided into a treatment group(43 cases) and a control group(37 cases).The western medicine,sustained release tablets of naproxen and enteric-coated tablets of sulfasalazine were given in the control group for six months.On the basis of control group,Chinese herb cake made of Huatanhuoxue prescription was sticked at Pangguangshu(BL28),Shenshu(BL 23),Geshu(BL 17),Huantiao(GB 30).Pishu(BL 20),Guanyuanshu(BL 26),Dazhu(GV14),which last 4 hours per time,once in a week and totally six months were required.The Bath ankylosing spondylitis disease activity index(BASDAI).Bath ankylosing spondylitis function index(BASFI).erythrocyte sedimentation rate(ESR).C reactive protein were observed before and after treatment as well as adverse reaction during the treatment,also clinical efficacy in two groups was compared.Results The total effective rate was 81.4%(35/34)in the treatment group,which was superior to 59.5%(22/37) in the control group(P<0.05).After the treatment.VAS,BASDAI and BASFI were all decreased in two groups,which was more obvious in the treatment group(all P<0.05).The level of ESR and C reactive protein were both decreased in two groups,which had no differences between each other(P>0.05).Conclusion The acupoint sticking with Huatanhuoxue prescription combined wilh western medicine is safe and effective for ankylosing spondylitis,especially for relieving pain and morning stiffness,improving spine function,which is worthy of further study and popularization and utilization.
