探讨阴式全子宫切除术的麻醉方法及其临床效果

目的观察分析不同麻醉方法应用于阴式全子宫切除麻醉的临床效果。方法选取150例2011年12月～2012年12月期间于我院进行阴式全子宫切除术的患者,依据麻醉方法不同随机分成腰-硬联合麻醉、硬膜外麻醉及骶管麻醉三组,每组各50例患者,观察麻醉效果。结果与硬膜麻醉比较,腰-硬麻醉与骶管麻醉的麻醉起效时间短、手术持续的时间较短、术中出血量较少、阻滞完善时间较短、镇痛效果好及肌松效果好,比较差异显著,p<0.05;腰-硬麻醉与骶管麻醉在阻滞完善时间、手术持续时间及出血量方面对比无显著差异,p>0.05。与CSEA、EA比较,骶麻组并发症率最低。结论在子宫全切除手术中,采用腰-硬麻醉与骶管麻醉的麻醉方法,起效时间相对较快,同时镇痛效果较好,可在临床广泛应用。

objective:to observe the different anesthesia analysis method is applied to Yin type whole hysterectomy anesthesia clinical effect.Methods:the 150 cases in December 2011 to December 2012 period in our for Yin type hysterectomy patients,according to the different methods of anesthesia at random into waist-hard combined anesthesia and epidural anesthesia and sacral canal anesthesia three groups,each group of each fifty patients,observe the anesthetic effect.Results:compared with hard film anesthesia,waist-hard anesthesia and sacral canal anesthetic anesthesia work time is short and operation for less time,amount of bleeding during the operation,the less perfect,block less time and analgesic effect is good and muscle loose effect is good,more significant difference,p < 0.05;Waist-hard anesthesia and sacral canal anesthesia in block perfect time,operation duration and amount of bleeding in contrast no significant difference,p > 0.05.And CSEA,EA comparison,sacral hemp group minimum complication rate.Conclusion:in the womb all surgery,the waist-hard anesthesia and sacral canal anesthetic anesthesia method,onset time relatively fast,and at the same time,analgesic effect is good,can be widely used in clinical.
