摘要
目的研究以宁夏两个"人人享有基本医疗卫生服务"政策试点市为样本,比较政策下卫生服务需方和供方,分析运行过程中存在的问题,为基本医疗服务的进一步实施和推广提供政策建议。方法对样本地区进行现场调查,采用一般描述性分析,t检验、χ2检验、TOPSIS法分析比较就诊人次、药事费收入与月份关系,项目运行前后满意度。结果项目实施后村民首选就医地点未发生变化,存在对药品种类及效果不满、不缴纳药事费现象,村卫生室均有药品未到位、药品代领、收入下降等现象。结论通过完善村卫生室配套制度,加强药品配送单位的督管,转变贵药,输液才能治病的观念,提高收入及简化工作量,加强基层医疗机构领导能力及村医医德建设等措施提高了项目满意度。
Objective Taking two towns as sample implementing the projects of 'everyone has a primary medical and health service', the residents of health services utilization and health service providers in different implementation regions was compared and the problems were analyzed for providing suggestions promoting the implementation to basic health care services. Methods By field survey, a descriptive method was adopted and t-test, χ2test and TOPSIS method was used to analyze and compare the frequencies of seeing doctors and expenses for medicine and monthly and satisfaction after and before implementing the project. Results After the project we found that the first doctor site did not change, people discontent the drug types and effects and not pay drug fee, the village clinic has short for drugs, drugs can get by anyone, and the income decline. Conclusion Through the method by improving village clinic supporting system, enhancing medicine management, changing medicine habit, strengthening basic health organizations leadership capacity-building and construction of medical ethics, the satisfaction of the project is increased.
