两种穿刺法在经桡动脉套管针穿刺置管中的对比观察

Comparing of two puncture methods of radial artery catheterization with arterial cannula

目的通过比较两种穿刺法的优缺点,寻求一种较为合理的经桡动脉套管针穿刺置管方法。方法 276例拟行经桡动脉套管针穿刺置管的病人随机分为A、B两组,每组138例。A组为透壁穿刺法,B组为直接穿刺法。均应用20G动脉套管针。观察两组病人的首次穿刺成功率、首次穿刺时间及失败例数。同时观察血肿等并发症的发生率。结果首次穿刺时间与最终失败例数两组差异无统计学意义,首次穿刺成功率A组高于B组(91.30%vs 82.61%,P<0.05)。两组并发症发生率差异无统计学意义。结论应用套管针行经桡动脉穿刺置管,透壁法优于直接法,不增加并发症,临床工作中可优先选择。

Objective To find a better way of radial artery catheterization with arterial cannula by comparing the two methods of transmural and direct arterial puncture catheterization.Methods A total of 276 patients underwent radial artery catheterization were randomLy allocated to two groups(Group A and Group B) with each group of 138 patients. Group A adopted the methods of transmural arterial puncture for catheterization and Group B adopted the methods of direct radial artery puncture for catheterization.Both group used 20G arterial cannula.Time and achievement ratio of first puncture,final failure in catheterization in each group were recorded.Complications of two groups were also recorded. Results There was a more higher achievement ratio of first puncture in Group A than in Group B(91.30%vs 82.61%, P<0.05).There was no significant difference of the complications between the two groups.Conclusion Transmural arterial puncture catheterization is better than direct radial artery puncture catheterizatio,which is the best choice when radial artery catheterization using arterial cannula needed in clinic.
