摘要
胃幽门杆菌(Helicobacter pylori)是主要感染人类胃部并引起胃溃疡、十二指肠溃疡,也与胃癌息息相关的病原菌。目前在治疗上以抗生素为主,但由于细菌抗药性问题,以及治疗所延伸之副作用,使得抗生素疗法仍有很大改善空间。过去研究发现特定乳酸菌可以抑制胃幽门杆菌,故本研究尝试筛选具有高度抑菌能力之菌株。本研究利用纸锭扩散实验证实乳酸菌发酵上清液可以抑制胃幽门杆菌的生长,且筛选出来的乳酸菌抑菌效果优于商品化乳酸菌株。从50株乳酸菌中筛选出抑制幽门杆菌效果最佳的前三株乳酸菌,分别为Lactobacillus acidophilusLA27、Lactobacillus pentosusLPS16、与Lactobacillus plantarumLP23,进行进一步的研究。实验证实这3株乳酸菌除可以抑制一般性胃幽门杆菌外,还可以抑制多重抗药性之胃幽门杆菌,且其对十二指肠溃疡菌株之抑菌能力优于胃癌菌株。流式细胞仪分析也证实这3株乳酸菌可吸附到胃细胞AGS与MKN45,显示这3株乳酸菌有更大的机会停留在胃部产生抑菌效果。进一步以C57BL/6公鼠进行动物实验,结果发现,先喂食乳酸菌再感染幽门杆菌,3株乳酸菌皆能在幽门杆菌感染后3周或6周内,于Antrum、Body、或整体胃部中改善幽门杆菌造成的急性或慢性发炎反应。因此,我们推论乳酸菌LA27、LPS16与LP23三株菌具有减缓幽门杆菌感染与降低因感染而引起发炎反应的能力。
Helicobacter pylori( HP) is the major pathogen of human stomach and is highly related to gastric ulcer, duodenum ulcer and stomach cancer. The current guidelines suggest antibiotics as the first choice therapy. Nevertheless,drug-resistance and side effects cause low eradication rate of current dogma and leave room for improvement.Studies in the past decade have demonstrated some lactic acid bacteria could inhibit HP. In this study we try to screen the potential probiotic candidates. Disc diffusion assay was used for screening and found only the supernatant of cultured broth,not the cell,could inhibit HP and many of them showed better inhibitory activity than commercial positive control. Top three were selected for further studies from the screened 50 strains and were identified as Lactobacillus acidophilusLA27、Lactobacillus pentosusLPS16、and Lactobacillus plantarumLP23 respectively. These three strains could inhibit not only general HPs but also multi-resistant strains and showed more inhibitory effect on HPs from the duodenum ulcer than from the gastric cancer. Good adhesive ability to gastric cell lines AGS and MKN45 were also demonstrated by flow cytometry. Feed probiotics to C57BL /6 male mice before HP infection showed reduced acute and chronic inflammation at antrum and body at week 3 and 6. Both in vitro and in vivo studies provide evidence that probiotics LA27,LPS16 and LP23 could reduce the HP infection and alleviate gastric inflammatory responses and may represent a novel approach to the management of HP infection.
