AngleⅡ1患者治疗前后颅颌骨及牙性测量指标变化的临床研究

Clinic study of the skull,jaw and teeth measurements of ClassⅡdivision 1 malocclusion before and after treatment

摘要 目的:研究分析AngleⅡ1患者治疗前后颅颌骨及牙性测量指标的变化。方法选取2009至2013年沈阳军区总医院收治的恒牙牙合AngleⅡ1深覆盖(Ⅰ、Ⅱ°)患者30例,应用固定直丝弓技术配合口外弓、Ⅱ类牵引进行非拔牙矫治。其中A组15例患者使用口外弓配合Ⅱ类牵引治疗,B组15例患者仅使用Ⅱ类牵引治疗。比较两组治疗前后颅颌骨及牙齿12项测量值,进行统计学分析。结果 A组:蝶鞍点-鼻根点-上牙槽座点角(SNA角)、上牙槽座点-鼻根点-下牙槽座点角(ANB角)、Wits、上中切牙长轴-前颅底平面交角(Ul-SN角)、上中切牙长轴-NA连线交角(U1-NA角)、Overjet、Overbite治疗后均减小,差异均有统计学意义;蝶鞍点-鼻根点-下牙槽座点角(SNB角)、下中切牙长轴-下颌平面交角(L1-MP角)、下中切牙长轴-NB连线交角治疗后均增大,差异均有统计学意义。B组:ANB、Wits、Overjet、Overbite治疗后均减小,差异均有统计学意义;SNB、L1-MP、L1-NB治疗后均增大,差异均有统计学意义。两组治疗前后差值的组间比较均无统计学意义。结论使用口外弓治疗Ⅰ、Ⅱ°深覆盖患者,不仅能使患者上颌牙后移,前牙覆盖减小,而且上颌骨向前的生长可以得到抑制,上颌前突得到改善。 Objective To investigate skull,jaw and teeth measurements of ClassⅡdivision 1 malocclusion before and after treatment. Methods 30 patients with Class Ⅱdivision 1 malocculsion were included,the headgear coordinated with Class Ⅱelastics were used in group A (15 patients);while group B were treated with Class Ⅱelastics only(15 patients). 12 measurements were compared between A and B before and after treatment, and the results were analyzed by statistics. Results SNA、ANB、Wits、Ul-SN 、U1-NA、Overjet、Overbite decreased, while the angle of SNB、L1-MP 、L1-NB increased after treatment in group A,the difference was statistically significant. Otherwise, there was statistically significant decreased in ANB、Wits、Overjet、Overbite and increased in SNB、L1-MP、L1-NB of group B after treatment. The differences were not statistically significant between group comparison. Conclusion Treatment the malocclusion of Ⅰ、 Ⅱ degree overjet used with headgear not only acquire the distal movement of upper teeth and reduce the overjet , but also inhibit the maxillary development,improve the maxillary prognathism.
