自体双股半腱肌和股薄肌横向滑杆固定重建膝关节前交叉韧带的初步报告

A preliminary report of Transfix anterior cruciate ligament reconstruction using autogenous hamstring tendons: a new non-arthroscopic method

摘要 目的 介绍自体双股半腱肌和股薄肌横向滑杆固定重建膝关节前交叉韧带(ACL)的手术方法,并探讨其近期的临床效果.方法 自2005年1月~2006年7月,应用自体双股半腱肌和股薄肌横向滑杆固定治疗8例ACL损伤患者,其中2例为急性损伤,6例为慢性膝关节不稳.手术从健侧取半腱肌和股薄肌对折4股重建ACL;先建立"胫-股"隧道,然后在瞄准器帮助下采用拉出钢丝法使移植肌腱进入隧道,进入后肌腱股骨侧采用横向滑杆固定,胫骨侧采用界面挤压螺钉固定.术后3、6和12个月进行随访.结果 7例膝关节活动度完全恢复正常,1例患者膝关节伸直达180°,但屈曲为110°.7例Lachman试验较术前有明显改善,患者对膝关节的稳定性满意,已逐渐恢复运动能力;1例Lachman试验为Ⅱ度,患者对膝关节的稳定性基本满意.结论 自体双股半腱肌和股薄肌横向滑杆固定重建ACL具有操作方便、微创及重建ACL坚强牢固等优点.此技术对医生的限制低,具有推广价值.
