摘要
目的 :探讨分析应用替吉奥对晚期结直肠癌患者进行姑息化疗的临床疗效。方法 :选取2010年2月至2014年2月间我院收治的低体能评分晚期结直肠癌患者112例作为研究对象,按照治疗方案的不同将其分为FL组(65例)和S-1组(47例),采用FL(Mayo Clinic)方案为FL组患者进行治疗,应用替吉奥(S-1)为S-1组患者进行治疗,观察对比两组患者的临床疗效及不良反应的发生情况,并将对比的结果及两组患者的临床资料进行回顾性的分析。结果 :在FL组65例患者中,临床疗效判定等级为CR的患者有0例,临床疗效判定等级为PR的患者有6例,临床疗效判定等级为SD的患者有31例,临床疗效判定等级为PD的患者有24例,治疗的总有效率为4.7%,疾病控制率为60.66%;在S-1组47例患者中,临床疗效判定等级为CR的患者有0例,临床疗效判定等级为PR的患者有8例,临床疗效判定等级为SD的患者有29例,临床疗效判定等级为PD的患者有10例,治疗的总有效率为16.7%,疾病控制率为78.72%。S-1组患者的近期疗效明显优于FL组患者,差异显著(P<0.05),具有统计学意义。在两组患者的不良反应统计序列中均未出现4级不良反应。在1~3级不良反应中,除呕吐症状以外,S-1组患者白细胞减低、贫血、恶心、乏力、腹泻等不良反应的发生率均明显高于FL组患者,差异显著(P<0.05),具有统计学意义。结论 :应用替吉奥对晚期结直肠癌患者进行姑息化疗的临床疗效显著,疾病控制率较高,患者的耐受性也较好。但与FL化疗方案相比,患者发生消化道不良反应的几率相对较高。
Objective To compare the survivability of lower Karuafsky advanced rectal cancer patients between the cosmetic treatment S-1 single and FL. Methods A total of 112 advanced rectal cancer patients with lower Karuafsky collected from February 2010 to February 2014 in our department were divided into two groups: One group about FL with 65 patients, the other group about S-1 with 47 patients, twenty-one days was a circulatory period. The short-term efficacy and toxicity of two groups were evaluated by two circulatory period later. Results The both have different toxicity with bone marrow depression and digestive toxic reactions, There was no statistic significance about the incidence of vomit in the two group (P=0.224).while the incidence of grade 1-3 in S-1 group higher than group FL such as leucopenia, anemia, nausea fatigue, diarrhea(P<0.05). Conclusion Both the Group FL and S-1 have acceptable toxicity profiles while treat with lower Karuafsky advanced rectal cancer patients, but there were lower happened rate of digestive toxic reactions in group FL ,compare to S-1, the cosmetic treatment FL will be better for the survivability and economic, but the disease control rate may be even higher in group S-1 .
