医改前后农村老年家庭灾难性卫生支出状况分析

Analysis on Catastrophic Health Expenditure on Rural Elderly Families before and after Medical Reform

目的:分析医改前后农村老年家庭灾难性卫生支出情况。方法:通过入户调查分别收集2008年和2013年的家庭收入和支出情况、慢性病患病状况、卫生服务利用数据分析灾难性卫生支出发生率以及平局差距。结果:农村老年家庭灾难性卫生发生率和平均差距随着阈值和收入的升高而下降。与2008年相比,2013年农村老年家庭灾难性卫生发生率和平均差距上升,其中低收入和有住院发生的家庭上升明显,而高收入和无住院发生的家庭的略有下降。结论:医改缓解农村老年家庭灾难性卫生支出的作用有限,缺乏公平性。

Objective:MTo analyze the situation of catastrophic health expenditure on rural elderly families before and after medicalreform.ethods:Through household survey, family income and expenditure situation, chronic illness and health service utilization da-ta in 2008 and 2013 were collected to analyze the incidence and average gap of catastrophic health expenditure.Results:With the in-crease of the standard and income, there was a decrease in the incidence and average gap of catastrophic health expenditure in ruralelderly families. Compared with the data of 2008, the incidence and average gap of rural elderly family catastrophic health expenditureincreased in 2013, these indexes of families with low income and had hospitalization incidence increased, while these of families withhigh income and non hospitalization decreased.Conclusion:Medical reform had limited effect on relieving the catastrophic health ex-penditure on rural elderly families, and the inequity was insufficient.
