摘要
目的探讨新诊断2型糖尿病患者血浆中性粒细胞明胶酶相关脂笼蛋白(LCN2)浓度与颈总动脉内膜中膜厚度的关系。方法共纳入166例年龄在35~70岁之间、病程在1年以内的新诊断2型糖尿病患者,同时选取同期的65例健康正常人作为对照。采用高分辨超声检测2型糖尿病患者颈总动脉内膜中膜厚度,用双抗体酶联免疫吸附法检测血浆LCN2浓度。结果校正年龄、性别之后,2型糖尿病组的血浆LCN2浓度高于正常对照组(P<0.05)。将2型糖尿病组按照血浆LCN2浓度三分位分组,高浓度LCN2组颈总动脉内膜中膜厚度高于低浓度LCN2组(P=0.026)。在校正多个因素后,血浆LCN2浓度与颈总动脉内膜中膜厚度呈正相关(r=0.197,P=0.007)。新诊断2型糖尿病患者的血浆LCN2浓度是其颈总动脉内膜中膜厚度增厚的相关因素之一。结论新诊断2型糖尿病患者血浆LCN2浓度高于正常人,其血浆LCN2浓度与颈总动脉内膜中膜厚度呈正相关关系。血浆LCN2浓度上升可能是新诊断2型糖尿病患者颈总动脉内膜中膜厚度增厚的危险因素之一。
Aim To investigate the relationship between the concentration of plasma lipocalin-2( LCN2) and common carotid artery intima-medial thickness( CCA-IMT) in newly diagnosed type 2 diabetic patients. Method 166 newly diagnosed type 2 diabetic patients( age 35 ~ 70,duration≤1 year) and 65 healthy people were recruited. CCAIMT was detected by high-resolution ultrasonography,plasma concentration of LCN2 was measured by enzyme-linked immunosorbent assay. Results Adjusting for age and sex,type 2 diabetics' plasma concentration of LCN2 was significantly higher than that in the healthy group( P < 0. 05). According to the three tilter of plasma concentration of LCN2,the highest concentration group's CCA-IMT was significantly higher than that in the lowest concentration group( P = 0. 026).In the multi-variables partial correlation analysis,adjusting for age,sex,body mass index( BMI),waist circumstance( WC),waist-to-hip ratio( WHR),diastolic blood pressure( DBP),hemoglobin A1c( Hb A1c),fasting insulin( FIN),HOMA-IR,triglyceride( TG),low density lipoprotein cholesterol( LDLC),the positive relationship between circulating LCN2 and CCA-IMT was observed( r = 0. 197,P = 0. 007). Multiply linear regression analysis demonstrated that circulating LCN2 contributed to elevation of CCA-IMT in newly diagnosed type 2 diabetic patients. Conclusion Plasma concentration of LCN2 was much higher in newly diagnosed type 2 diabetic patients than that in healthy people. Plasma concentration of LCN2 was positively correlated with CCA-IMT in newly diagnosed type 2 diabetic patients. Elevation of plasma concentration of LCN2 was probably one of the independent risk factors contribute to the elevation of CCA-IMT in newly diagnosed type 2 diabetic patients.
