绝经前女性冠心病患者临床危险因素分析

Clinical Analysis of the Risk Factors in Premenopausal Women with Coronary Heart Disease

目的探讨绝经前女性冠心病临床危险因素特点。方法选择2013年10月至2014年5月拟诊冠心病住院的绝经前女性患者60例,根据冠状动脉造影检查结果分为冠心病组35例,平均年龄(47.6±3.8)岁,正常对照组25例,平均年龄(46.2±4.9)岁,均抽血检测总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、尿酸、血常规,并详细记录年龄、身高、体重、冠心病家族史、高血压病史、收缩压、舒张压及糖尿病史。结果冠心病组中早发冠心病家族史、高血压病史、糖尿病史、收缩压、甘油三酯、尿酸、中性粒细胞、单核细胞计数均高于正常对照组(P<0.05),高密度脂蛋白胆固醇、血红蛋白低于正常对照组(P<0.05);多因素Logistic回归分析提示糖尿病史、收缩压、甘油三酯、尿酸升高、血红蛋白降低为绝经前女性冠心病的独立危险因素,而高密度脂蛋白胆固醇为绝经前女性冠心病的保护因素。结论糖尿病史、收缩压、甘油三酯、尿酸升高、血红蛋白降低为绝经前女性冠心病的独立危险因素,而高密度脂蛋白胆固醇为绝经前女性冠心病的保护因素。

Aim To investigate the clinical features of the risk factors of premenopausal women with coronary heart disease( CHD). Methods From October 2013 to May 2014,60 premenopausal women patients diagnosed as coronary heart disease were included. According to the coronary angiography( CAG) result,all patients were divided into CHD group( 35 patients) and control group( 25 patients). All of the 60 cases were measured to determinate the level of total cholesterol( TC),triglycerides( TG),high density lipoprotein cholesterol( HDLC),low density lipoprotein cholesterol( LDLC),blood uric acid( UA) and routine blood,age,weight,family history of coronary heart disease,hypertension history and diabetes history in details. Results Compared with the control group,there were significant higher proportion of hypertension,family history of coronary heart disease,diabetes,systolic blood pressure( SBP) and increased TG,UA,neutrophils,monocytes levels( P < 0. 05) and decreased HDLC,hemoglobin( Hb) level in CHD group. Multivariate Logistic regression analysis showed diabetes,TG,UA,SBP and decreased HDLC were independent risk factors for CHD group patients. Conclusions The study suggests that TG,hypertension,SBP,diabetes,UA and lower HDLC levels were independent risk factors for premenopausal women with coronary heart disease.
