应用事件相关电位P300评估精神分裂症患者智能状态

Evaluation on the intellectual status of schizophrenia patients by using P300 event related potentials

目的:探讨精神分裂症患者P300电位与智商的变化,判定P300电位在评价此类患者智力方面的作用。方法:对40例精神分裂症患者和38例健康者分别进行了P300电位测定和韦氏成人智力量表(WAIS-RC)评定。结果:精神分裂症患者的P300电位波潜伏期(ms)延长(354.7±28.9),波幅(μV)降低(2.2±1.6),与对照组相比差异有显著性意义(t=2.58,P<0.01),WAIS-RC量表测验发现言语智商、操作智商及总智商均低于对照组,有显著性差异(t=2.65,2.72,3.10,P<0.01)。相关分析显示P300波潜伏期与各智商呈显著负相关(r=-0.28～-0.33,P<0.05),波幅与言语智商及总智商呈显著正相关(r=0.29～0.32,P<0.05)。结论:P300电位在评价精神分裂症患者智能方面具有重要意义,可作为一项客观的电生理学指标应用于临床。

AIM:To explore the changes of P300 potentials and intelligence in schizophreni a patients,and to evaluate the effect of P300 potentials in appraising the intel ligence of schizophrenia patients. METHODS:Forty schizophrenia patients and 38 healthy subjects were evaluated by P300 potentials test and Wechsler adult intelligence scale(WAIS-RC) respective ly. RESULTS:Latent period(ms) of P300 potentials prolonged(354.7±28.9),and amplit ude(μV) of P300 potentials lowered(2.2±1.6) in schizophrenia patients,signific antly different from those in the control group(t=2.58,P< 0.01).The results of W AIS-RC showed that verbal intelligence quotient(VIQ),performance intelligence q uotient(PIQ) and full intelligence quotient(FIQ) in schizophrenia patients were all significantly lower than those in the control group(t=2.65,2.72,3.10,P< 0.01 ). Correlation analysis showed that there was a remarkable negative correlation of P300 latency and VIQ,PIQ and FIQ respectively(r=-0.28--0.33,P< 0.01),and a significant positive correlation between P300 amplitude and VIQ and FIQ respect ively(r=0.29-0.32,P< 0.05). CONCLUSION:P300 potentials are very important in evaluating the intelligence o f schizophrenia patients,and can be used clinically as an objective index of ele ctrophysiology.
