恶性青光眼治疗的临床研究

Clinical research on therapy of malignant glaucoma

目的:探讨恶性青光眼的治疗方法并评价其疗效。方法:对17例(17眼)恶性青光眼不同治疗方法及手术时机进行回顾性分析。结果:4例药物治疗,晶状体囊内摘除+玻璃体前界膜划开4例,晶状体囊外摘除+后囊膜切开+前部玻璃体切除9例,17例前房均恢复并保持良好。结论:恶性青光眼是多种发病因素混合存在的难治性青光眼,早期药物治疗有效,后期多种联合手术则是最有效的方法。

AIM: To explore the therapeutical schemes available for malignant glaucoma and to evaluate the effects.· METHODS: Different therapeutical schemes and operational occasion were retrospectively analysed in 17 cases (17 eyes) of malignant glaucoma.· RESULTS: All anterior chambers were reconstructed and kept well. Of all the 17 cases, 4 were medicated, 4 underwent intraocular lens extraction and incision of vitreous anterior limiting membrane and 9 underwent extraocular lens extraction, posterior membrane incision and anterior vitreous resection.· CONCLUSION: Malignant glaucoma is refractory glaucoma with many pathogenetic factors, on which medication is effective in early period, while multiple surgery should be used in advanced stage.
