角膜碱烧伤早期应用皮质类固醇疗效观察

Effect of early use of topical corticosteroid in corneal alkali burn.

目的 探讨角膜碱烧伤早期应用皮质类固醇治疗的效果。方法 对 11例 ( 18眼 )角膜碱烧伤后立即应用地塞米松 2mg结膜下注射 ,隔日 1次 ,连用 3次。停用地塞米松 3周后 ,再加用地塞米松 2mg结膜下注射 ,隔日 1次 ,连用 3次。同时给维生素C 3g静脉滴注 1日 1次。结果 Ⅰ～Ⅱ度 8例 ( 13眼 )用药后第 2天 ,刺激症状明显改善 ,角膜内皮水肿明显好转 ,1月后复查视力 0 .4～ 0 .8。Ⅲ度 3例 ( 5眼 )用药后刺激症状逐渐消失 ,角膜内皮水肿逐渐减轻 ,1月后复查视力 0 .1～ 0 .5。结论 角膜碱烧伤早期应用皮质类固醇 ,避开 2～ 3周危险期 。

Objective To study the effect of early use of topical corticosteroid in alkali burned corneas.Methods 18 eyes of 11 cases which suffered from alkali burn were administered immediately by dexamethasone sodium phosphate atdose of 2 mg every other day for 3 successive times besides systemic ascrobate. The same protocol was repeated after three weeks following last dose of dexamethasone. Results Inflammatory symptoms and corneal edema resolved immediately in 8 cases with grade I-II after the second day, which achieved the final visual acuity of 0.4-0.8 after 1 month. 3cases with grade III,the inflammatory symptoms and corneal edema improved gradually. Their outcome of visual acuity was 0.1-0.5 after 1month. Conclusions Use of topical corticosteroids in alkali burn in early in the first week was effective and safe.
