经导管动脉介入溶栓治疗下肢深静脉血栓的疗效分析

Clinical research of artery catheterization thrombolysis in treating deep venous thrombosis of the low extremity

目的评价经动脉途径应用溶栓药治疗下肢深静脉血栓(DVT)形成的临床疗效。方法采用Seldinger法,经健侧股动脉插管到患侧动脉周围加压灌注溶栓药,在应用溶栓药同时给予抗凝治疗。结果治疗组症状改善率100%,治愈率83.3%;对照组症状改善率75%,治愈率35%,治疗组明显优于对照组(P<0.01)。从治疗前后肢围的比较,治疗组明显优于对照组(P<0.05),从时机来看,导管介入法即适用于早期又适用于晚期重症者。消肿时间治疗组均≤7d,与对照组差异有显著性。结论经导管动脉内途径应用溶栓药物治疗下肢DVT疗效显著,值得临床推广,同时应重视抗凝治疗在DVT治疗中的重要性。

Objective To investigate the clinical curative effect on the lower extremity deep venous thrombosis by using artery catheterization thrombolysis. Methods With Seldinger′s technique, catheter was placed into popliteal artery through the contralateral healthy side femoral artery and then the thrombolytic and anticoagulant agents were dripped into the thrombotic popliteal artery. Results In the treatment group, symptoms improved in all patients(100%), disappeared in 25 patients(83.3%). In the control group, symptoms improved in 15 patients(75%), disappeared in 7 patients(35%). The differences were significant (P< 0.01). The comparison of limb parameters pre and post treatment was obviously excelled to the control group(P< 0.05). The treatment was not only suitable in early stage but also as well for the serious ones in advanced stage. In the treatment group, the time for the swell of the low extremity to subside was≤7 days, the difference was significant compared to the control group. Conclusions Arterial catheterization thombolysis is an effective method in treating deep venous thrombosis of the lower extremity, anticoagulant agents is important in treating deep venous thrombosis of the lower extremity .
