卡介菌多糖核酸对不同来源淋巴细胞膜黏附分子的影响

摘要 目的为考察卡介菌多糖核酸辅助治疗结核病的细胞学机制,并比较在不同病理情况下作用的差异。方法分别选用正常人、新发浸润型病灶为主肺结核患者、长病程纤维空洞型病灶为主肺结核患者以及结核性胸膜炎患者的外周血分离淋巴细胞。采用酶联免疫吸附试验(ELISA)法检测其膜表面黏附分子CD18的表达,并进一步检测上述细胞经与卡介菌多糖核酸共孵育24h后表达的改变情况。结果浸润型结核和结核性胸膜炎患者外周血淋巴细胞的CD18呈高水平表达,与正常人相比差异有显著性(P<0.01);纤维空洞型结核患者外周血淋巴细胞CD18的表达水平则与正常人差异无显著性(P>0.05)。卡介菌多糖核酸可显著提高后者CD18的表达水平,但对巳呈高表达的前两者无显著影响。结论卡介菌多糖核酸可刺激激活纤维空洞型结核患者外周血淋巴细胞,改变其活性低下状态,有利于结核病灶的控制,但对已呈高度活化的浸润型结核和结核性胸膜炎患者则无同样益处。 Objective To study the mechanism of siqikang injection used to help treat patients with tuberculosis. Methods The normal peripheral blood lymphocytes were collected from normal human,patients with newly infiltrative active and chronic fibrotic cavitary pulmonary tuberculosis , patients with tuberculous pleurisy respectively. The expression of adhesive molecule CD18 upon the lymphocyte membrane and its changes after 24 h of incubation with siqikang were observed by ELISA. Results The CD18 expressive levels of the lymphocytes from patients with infiltrative active tuberculosis and tuberculous pleurisy were significantly higher than those on the lymphocytes from normal human(P<0.01). The CD18 expressive level of the lymphocytes from patients with chronic fibrotic cavitary tuberculosis was identical with that on the lymphocytes from normal human approximately(P>0.05); but it could be greatly increased by siqikang stimulation.However siqikang had little effect on the CD18 expression of the lymphocytes from the patients with infiltrative active tuberculosis and tuberculous pleurisy. Conclusion Siqikang injection can activate the peripheral blood lymphocytes from patients with chronic fibrotic cavitary tuberculosis,and alter their low active estate ,so it is helpful to control the tuberculous focus ,but hasn′t more benefit to patients with infiltrative tuberculosis or tuberculous pleurisy, whose lymphocytes have already been highly activated.
