21℃海水浸泡对火器伤合并失血休克犬血液流变学和血气的影响 被引量:1

Effects of 21℃ seawater immersion on hemorhology and blood gas in dogs with fire-arm injury combined with hemorrhagic shock

摘要 目的探讨21℃海水浸泡对火器伤合并失血性休克犬血气及血液流变学等指标的变化及规律。方法健康成年雄性犬21条,随机分为陆地创伤休克组(n=10),海水浸泡创伤休克组(n=11)。应用滑膛枪发射钢珠弹射击动物右侧后肢,然后左侧股动脉快速放血使血压降至40mmHg(5.3kPa),维持1h,造成创伤失血性休克模型。动物分别置于21℃人工海水和陆地观察,测定伤前、海水浸泡或陆地观察(1,3,5)h血液流变学、血气指标及动物活存时间。结果海水浸泡创伤休克组动物存活率显著低于陆地创伤休克组,血液流变学及血气指标均较陆地创伤休克组恶化。结论血液流变学和血气改变在海水浸泡火器伤合并失血性休克动物的高死亡率中起着重要的作用。 Objective: To investigate the effects of 21℃ seawater immersion on the parameters of hemorhology and blood gas in dogs with fire-arm injury subjected to gun shot combined with hemorrhagic shock. Methods: 21 male dogs were divided into land traumatic shock group (n=10), seawater immersion traumatic shock group (n=11) randomly. The dogs′ right thighs were shot with steel balls by a smooth-bore rifle, and then were bled from left femoral artery until the mean arterial pressure (MAP) to 40mmHg (5.3 kPa), the MAP were maintained for 1 hour, formed traumatic shock model. Then dogs were immersed in 21℃ seawater or placed at the room temperature. The parameters of hemorhology and blood gas were measured at preshock, 1, 3, 5 hours after immersed in 21℃ seawater or 21℃ room temperature. The survival rate and survival time of dogs were observed. Results: The survival rate of the seawater immersed traumatic shock group was lower than that of the land traumatic shock group significantly. The parameters of hemorhology and blood gas in seawater immersed traumatic shock group were worse than those in the land traumatic shock group. Conclusions: The changes of hemorhology and blood gas were important cause for death in dogs with fire-arm injury combined with hemorrhagic shock.
