外伤性听骨链中断的诊断与治疗(附6例报告)

Diagnosis and treatment of traumatic ossicular chain disruption

目的探讨外伤性听骨链中断的诊断和治疗。方法对6例听骨链中断的病人进行手术重建听骨链,并进行术后随访。结果术后随访6个月至1年,气导听力提高20dB的3例;提高16郾3dB的1例;提高10dB的1例;1例在出院时听力提高了23郾3dB,1月后听力下降,考虑听骨链又有中断。结论听骨链中断的病人,在头、耳部外伤治疗一月后仍呈传导性耳聋,应考虑听骨链中断,即行手术探查。手术中根据具体情况,重建听骨链。

Objective To explore diagnosis and therapy of traumatic ossicular chain disruption. Methods 6 cases with ossicular chain disruption were operated on and the ossicular chain was reconstructed. All the cases were followed up. Results During six months to one year of follow-up, the air-conducting hearing levels had an enhancment of 20dB in 3 cases,15.3 dB in 1 case,10 dB in 1 case,and 23.5dB in 1 case,but hearing of the last case declined 1 month postoperatively. The patient suffered from ossicular chain disruption again. Conclusion The patients with conduction deafness which has lasted for one month after head and ear trauma shoud be considered in relation to ossicular chain disruption. The middle ear had to be explored and ossicular chain reconstruction will be performed on a case-by-case basis.
