伸展体位与短节段椎弓根螺钉治疗胸腰椎爆裂性骨折

Surgical treament of thoracolumbar burst fracture with extension reduction and short-segment pedicle screw system fixation

目的 探讨应用伸展体位加短节段椎弓根螺钉系统治疗胸腰椎爆裂性骨折的效果。方法 采取肢体伸展体位使伤椎间接复位、椎弓根螺钉系统固定治疗胸腰椎爆裂性骨折 2 7例。结果 随访 3～ 2 3个月 ,伤椎前、后高度由术前的平均 4 7 3%、6 3 5 %恢复到术后正常高度的 90 2 %、94 5 % ;后凸Cobb角由术前平均2 1 6°恢复到术后平均 2 8°。结论 伸展体位加短节段椎弓根螺钉系统对胸腰椎爆裂性骨折的治疗复位确切、固定可靠、疗效良好。

Objective To investigate the effect of surgical treament for thoracolumbar burst fracture with extension reduction and short-segment pedicle screw system fixation. Methods 27 cases of thoracolumbar burst fractures were treated with reduction by extension posture and short-segment pedicle screw fixation system. Results All cases were followed up for 3～23 months. The anterior and posterior heights of compressed vertebral bodies were restored from 47.3% and 63.5% preoperatively to 90.2% and 94.5% of the normal height postoperatively. The kyphotic Cobb's angle was corrected from 21.6°preoperatively to 2.8°postoperatively on average. Conclusion The method of extesion reduction and short-segment pedicle screw system fixation demonstrates good reduction solid fixation and satisfactory outcome for thoracolumbar burst fractures.
