肺炎衣原体感染与急性心肌梗死、血栓素B_2、6-酮-前列环素F_(1α)、P-选择素的关系

摘要 目的 探讨肺炎衣原体 (Cpn)感染与冠心病心肌梗死的关系。方法 测定 5 1例急性心肌梗死 (AMI)和 4 2例陈旧性心肌梗死 (OMI)患者及 31例冠脉造影正常者 (NC)血Cpn抗体水平及DNA ,同时观测 6 -酮 -前列环素F1α(6-Keto -PGF1α)、血栓素B2 (TXB2 )、P -选择素 (GMP1 40 )变化及其与Cpn感染的相关性。结果 AMI组CpnIgG、CpnIgM阳性率及水平高于NC组 (P <0 .0 5 ) ,CpnDNA检测结果与之吻合。校正冠心病危险因素前、后 ,CpnIgG阳性与AMI均有相关关系 (OR 3.6 5 3,P =0 .0 2 5 ;OR 3.174 ,P =0 .0 33)。AMI组中Cpn(+)组TXB2 、GMP1 40 高于Cpn(- )组 ,而 6 -Keto -PGF1α低于Cpn(- )组 (P均 <0 .0 5 )及NC、OMI组中Cpn(+)组 (P均 <0 .0 1) ;且IgG与TXB2 、GMP1 40呈正相关 ,与 6 -Keto -PGF1α呈负相关。结论 Cpn感染与AMI之间存在明显的相关性 ,与TXB2 、6 -Keto -PGF1α及GMP1 40 Objective To investigate the correlations between chlamydia pneumoniae (Cpn) infection and acute myocardial infarction(AMI).Method Specific antibodies and DNA to Cpn were measured in 51 patients with AMI(AMI group),42 patients with old myocardial infarction (OMI group) and 31 normal controls (NC group) who had been excluded angiographically from coronary artery disease. Moreover,the changes and relationships between 6-Keto-prostaglandin-F 1α(6-Keto-PGF 1α),Thromboxane B 2(TXB 2),P-selection(GMP 140) and Cpn infection were determined in the three groups.Results Cpn and CpnDNA seropositive,and the average concentration(O.D.) of IgG(IgM) antibody to Cpn was significantly higher in the AMI group than that of the NC group.The correlation between the Cpn infection and AMI was strong(OR 3.653).Adjustment for the major cardiovascular risk factors didn't attenuate this correlation(OR 3.174). Those who were Cpn positive in the AMI group had significantly higher levels of TXB 2 and GMP 140,while significantly lower level of 6-Keto-PGF 1α not only than those who were Cpn negative in the same group but also than those who were Cpn positive in the NC group and OMI group.Correlation analysis revealed that the average concentration of IgG antibody to Cpn was positively associated with the levels of TXB 2 and GMP 140 but negatively associated with the level of 6-Keto-PGF 1α in the Cpn positive patients of the AMI group.Conclusion There is a significant correlation between Cpn infection and AMI.There is a certain correlation between TXB 2,6-Keto-PGF 1αa,GMP 140 and Cpn infection, too.
