超声心动图对急性期川崎病的诊断价值

Diagnostic Value of Echocardiography for Acute Kawasaki Disease

目的 探讨超声心动图对急性期川崎病的诊断价值。方法 回顾性分析 12 9例确诊川崎病患儿的超声心动图表现 ,重点观察冠状动脉有无扩张、管壁形态和管腔形态。结果 12 9例川崎病急性期患儿中冠状动脉正常 9例 (7.9% ) ,冠状动脉扩张 87例 (6 7.4 % ) ,冠状动脉管壁形态异常 114例 (88.4 % ) ,管腔形态异常 10 3例 (79.8% ) ,管壁形态异常和 /或管腔形态异常 115例 (89.2 % )。结论 冠状动脉扩张和冠状动脉管壁管腔异常是超声心动图检测急性期川崎病的两项重要指标 ,后者更具敏感性 ,而两者联合应用检测急性期川崎病冠状动脉损害的阳性率高达 93.0 % 。

Objective To explore the role of echocardiography in diagnosis of Kawasaki disease at acute stage.Methods The features of echocardiography of 129 patients with Kawasaki disease were analyzed retrospectively.Results Normal coronary arteries were found in 9 patients, dilation of coronary artery was in 87 (67.4%),wall echo abnormality was in 114(88.4%),and abnormal shape of coronary lumen was in 103 (79.8%). Wall echo abnormality or/and abnormal shapes of coronary lumen were found in 115 cases(89.2%),which was more sensitive index than the dilation of coronary artery to diagnose acute Kawasaki disease(χ 2=7.20,P=0.007).Conclusion Wall echo abnormality or/and abnormal shapes of coronary lumen, together with dilation of coronary artery, is an important index to diagnose Kawasaki disease at acute stage.
