摘要
目的:探讨 MRI动态增强及 FFE序列对垂体微腺瘤的诊断价值。方法:对 5 6例垂体微腺瘤先行矢状位及冠状位 T1 WI和 T2 WI成像 ,然后进行 GD- DTPA冠状 T1 WI动态增强扫描 ,随之行冠状 T1 WI常规延迟 FSE和 FFE序列增强扫描。 结果 :(1)平扫 :直接征象显示 2 8例 (5 0 .0 % ) ,间接征象出现最多为垂体上缘局限膨隆 ,占5 0 %。(2 )平扫显示 2 8例 (5 0 .0 % ) ,动态增强显示 5 5例 (98.2 % ) ,常规延迟增强显示 4 8例 (85 .7% ) ,三者检出率相比差异有统计学意义 (P<0 .0 0 5 )。 5例只有动态增强能显示。 (3) FSE序列显示 4 8例 (85 .7% ) ,清楚显示 2 6例 ;FFE序列显示 5 1例 (91.1% ) ,清楚显示 35例。两者检出率相比差异有统计学意义 (P <0 .0 5 )。结论:MRI动态增强扫描与常规延迟增强 (FFE序列 )扫描明显提高病变检出率 ,可以避免病变遗漏。
Objective: To evaluate MRI effectiveness of dynamic contrast enhancement and FFE sequence in the diagnosis of pituitary microadenomas. Methods: 56 patients with pituitary microadenoma were studied by means of GD-DTPA-enhanced dynamic MRI and were also scanned with fastspin echo(FSE) sequences and fast field echo (FFE) sequences before and after dynamic scanning. Results: (1)plain scan: direct signs showed 28 cases(50.0%). the diagnosing values of indirect signs varied, most indirect sign was 50%. (2) plain scan detected 28 cases(50.0%), dynamic scan detected 55 cases (98.2%), conventional contrast enhancement detected 48 cases(85.7%), contrast enhancement scan was superior to the plain scan (P<0.005). (3)FSE sequence showed 48 cases,FFE sequence showed 51 cases.FFE was superior to FSE (P<0.05). Conclusions: Dynamic and delayed conventional contrast enhancement(FFE sequence) improved lesion detection over plain MRI, avoid lesion miss.
