改良拦截劈裂技术在硬核白内障超声乳化手术中的应用

The application of the modified stop and chop technique for phacoemusification of hand nucleus cataract

目的 探讨改良拦截劈裂技术在硬核白内障超声乳化手术中的应用。方法 对硬核白内障 ( 、 级核 ) 92只眼 ,分别采用改良拦截劈裂和拦截劈裂两种方法进行手术。记录术中超声时间 (U S)和有效超声时间(EP)。观察术后第 3、7及 30天的视力和有关并发症。结果 术中改良拦截劈裂组的超声时间和有效超声时间均短于拦截劈裂组 ,两组相差显著 (P <0 .0 5 ) ,改良拦截劈裂组术后 3、7及 30天的视力≥ 0 .5者分别为 19只眼(38.0 % )、38只眼 (76 .0 % )及 4 4只眼 (88.0 % ) ,拦截劈裂组分别为 8只眼 (19.1% )、2 1只眼 (5 0 % )及 35只眼(83.3% )。主要并发症有角膜水肿和后囊膜破裂。结论 改良拦截劈裂技术具有超声时间短、并发症少、劈核更加容易 ,安全等特点 。

Objective The study was designed to investigate the application of the modifeid stop and chop method for phacoemusification of hard nucleus cataract.Methods The technique of modified stop and chop or stop and chop were performed in 92 eyes of hand nucleus cataract.Results The intrao-operative US and EP were less in the modified stop and chop group than in the stop and chop gorup.A significant difference existed comparison between the two groups (P<0.05).A postoperative 3 days,7 days and 30 days the visual acuity with correction 0.5 or better was obtained respectively in 19 eyes (38.0%),38 eyes (76.0%) and 44 eyes (88.0%) in the modified stop and chop group and 8 eyes (19.1%),21 eyes (50.0%) and 35 eyes (83.3%) in the stop and chop group.The major complication were corneal edema and posterior capsular rupture.Conclusion The modified stop and chop technique for hard nucleus cataract is worthy to be spread due to safety,easiness,less phaco time and minimum complication.
